The short answer
What Should Be Written on a Ukraine Insurance Certificate?
A Ukraine insurance certificate should show the exact name and passport number from the document you will present, cover the whole stay with correct dates, display the limit matching the chosen plan, and include a territory line that matches your itinerary. Verify each line before signing and again on the received certificate.

Which name should the certificate show?
The certificate should show the name exactly as the data page of your passport prints it: no nickname, no shortened work name, no letter added or dropped.
- Check the certificate against clause 1.8.2 of the public offer, which says the form is prepared from the terms and data you selected.
- Confirm that you compare the line with the data page before you sign, and again on the certificate you receive.
In which order should the names appear?
Follow the order the form asks for, with each name spelled as the passport shows it.
- Check that the family name and given name are not swapped and that a second given name is not dropped; if your passport shows no patronymic, ask the seller what to enter rather than inventing one.
- Confirm that you type each name into its own place from the data page and read the line back against the passport.
Which passport number should the certificate carry?
The passport number on the certificate should be the number of the passport you will hand over at the crossing.
- Check that the number is not from an old passport, a copy or a different document.
- Confirm that Article 8(1) of the Law of Ukraine On Border Control, No. 1710-VI, lists a valid passport document as the first condition for border crossing, and that the State Border Guard Service of Ukraine lists a foreign passport first among crossing documents on its general-requirements page dated 3 March 2025.
- Verify that you check every digit and letter of the number against the data page before you pay.
What if you renewed your passport or hold two?
Enter the number of the passport you will actually present, and decide which one that is before you buy.
- Confirm that you choose one passport for the whole trip and enter that number before you buy.
- Verify that after purchase you ask how a changed number is recorded; amendments are made by a supplementary agreement executed by the parties (clause 3.9 of the public offer).
How should a child on a parent's passport appear?
Enter the child's name and date of birth exactly as the document that records the child shows them.
- Check that the child is not left off the certificate and is not entered with the parent's details.
- Confirm that under paragraph 5(2) of Cabinet of Ministers of Ukraine Resolution No. 118 of 1 March 2017, a person entered in a foreigner's passport document, and travelling with that foreigner, files a separate visa application form.
- Verify that before payment you ask the seller how a child without a passport of their own is entered.
How are extra travellers listed on one policy?
List each extra traveller as a named insured person, and check each name and date of birth against that person's own passport.
- Check that no name is missing, misspelt or attached to another person's date of birth.
- Confirm that details are not taken from memory but from each passport.
- Verify that you put every passport on the table while filling the form and count the rows against the people travelling.
Which dates should the term line cover?
Choose dates that cover the whole stay, from the day you arrive to the day you leave.
- Check that the end date is not before your departure day and the start date is not after your arrival day.
- Confirm that dates are not picked for a shorter or later trip and that day and month are not entered incorrectly.
- Verify that you read the day and month twice against tickets and bookings and ask the seller to correct a wrong date before travel.
Which sum insured should the certificate show?
The sum insured is the limit printed on the certificate, and it should match the plan you chose: 100,000 UAH (≈ €2,000) for Standard or €30,000 for Extended.
- Check that the limit printed is not lower than the plan you intended to buy.
- For a visa application, paragraph 5(4) of Resolution No. 118 requires at least €30,000 coverage unless exempt. The Standard limit is below this, while the Extended limit matches it.
- Verify that you check the plan name and limit before signing and note that the sample lists the sum insured separately from the rate, payment and total.
What should you read on the territory line?
Read the territory line on your own certificate and compare it with the places on your route. This line should name the territory covered, so check that every city and region on your itinerary is included before you travel.
- Check that the territory line names the countries or regions on your route.
- Confirm that no place on your route lies in an area the line excludes.
- Verify that the territory matches the one printed on your certificate.
What should you check before you sign and travel?
Before you sign and travel, verify every line on the certificate against your documents and itinerary.
- Check that the area named on the certificate matches the cities on your route.
- Confirm that you list the places you plan to visit and read the line against that list.
- Check that the name and passport number match those on the passport you will carry.
- Confirm that each traveller has their own row with name and date of birth from their own document.
- Verify that the dates cover the whole stay.
- Ensure that the sum insured equals the limit of the plan you chose.
- Verify that the territory line matches the one printed on your certificate.
